Valley Health
Center Integrative Medicine is a fee for service practice. We accept
payment at the time of service by cash, check, or major credit card.
Third Party Insurance
In order for the doctors to devote the time and care that patients
desire, Valley Health Center Integrative Medicine is a
fee-for-service practice. This arrangement is an attempt to keep
costs down and to achieve some degree of freedom to treat patients
without the often-competing goals of insurance companies and of the
pharmaceutical industry.
To determine how
your insurance plan covers out-of-network visits, contact your
employer’s benefits office or call the phone number on your
insurance ID card. At your visit, the office will provide you with a
claim form for you to sign and mail to your insurance company. The
form will include all the information needed from the doctor for
your insurance company to process your claim.
Medicare and Medicaid
For our patients who are eligible for Medicare or Medicaid, we are a
“Participating Provider”. This means that we must accept their
allowed charge for the services rendered, writing-off the difference
between what is charged and the amount allowed. A check will be sent
directly to our office for 80% of the allowed amount; the patient is
responsible for 20% of the allowed charge plus any deducible. We ask
that the 20% and any outstanding deductible be paid at the time of
the visit. |
