All new patients at Valley Health Center must complete an information sheet prior to seeing one of the doctors. Please print and complete one of the forms below and bring it with you to your first appointment. Adult Patient Information Sheet Pediatric Patient Information Sheet Your medical history and treatment records are an important element necessary to provide the best care possible. If you choose to become a patient of Valley Health Center, we will assist you in transferring your medical records to our office. Due to the confidentiality of medical records, you must sign specific forms to accomplish this task. Our office staff can assist you in this process. |
